The way to unlock the quest: Automatically, after you complete the "Evil's Refuge" main quest.

This is the final quest of the game that unlocks automatically, after you complete the "Evil's Refuge" quest, i.e. at the moment at which you reach Nekroloth's hideout. After a short conversation, Nekroloth turns into the Titan of Death and the duel with the final boss starts. During the fight, you will be controlling the Nameless One's spirit.
NOTE - from the moment, at which you start this quest, YOU WILL NOT BE ABLE TO RETURN back outside. Apart from that, Risen 3 does not offer any New Game Plus mode so, after the end of the battle with the final boss, the game will be, simply, over. Therefore, make sure that you have saved the game, just in case, before the fight with Nekroloth and that you have completed all of the side quests, and also checked out on all the side activities (if you are interested in them).
Right after the fight starts, you will learn one of its most important features. The Titan of Death will be trying to feed on the motionless body of the protagonist, on a regular basis. You need to prevent him from doing that because, as a result of that, you will be losing your health points. Fortunately, to stop the process of feeding on the body, you only need one attack which, for at least a dozen-or-so seconds will discourage the boss from doing that.
After you attract the attention of the Titan of Death, you need to perform attacks with consideration, because for obvious reasons, the Titan's attacks are very strong. While at close quarters, the opponent will try using his blade and, at a longer range, he will be shooting magical green projectiles, towards the Nameless One. Just like in the case of all the other fights with mighty opponents, rely on dodging and parrying, while staying in constant motion. At the same time, search for opportune moments to launch your favorite attacks.
After the boss loses, about, a third of his health, the duel will be enriched with several additional elements. The Titan of Death will, from now on, mainly try to snatch the body of your character and attempt to feed on him in seclusion. To move faster around the cavern, use the crystal portals around - pass through them up until you locate the boss.
A much more serious handicap now is that monsters start appearing around. As much as eliminating the weak Hellhounds is always easy, the Soul Eaters are not that easy to kill. What is even more, they deal much more damage. In spite of that, I recommend that you eliminate the additional beasts only if they pose an immediate threat to the Nameless One, e.g. as a result of cornering him. The reason is that, after you kill the creatures, soon there appear new ones to replace them. This means that spending too much time on killing them makes the fight longer (and makes the risk of losing more real).
It is worth noting, at this point, that there is a major facilitation to the boss battle - in the central part of the cave, there is a spire of light shown in the above screenshot. Crossing that spire regenerates your character's health. This is a good alternative for the standard healing supplies. You can make the battle easier for you, by using spells like Summon Hellhound or Summon Shadow Warrior from rune magic. The summons have a visible input into distracting the creatures that accompany the boss.
In order to complete the final battle, you need to deprive the Titan of Death of the entire health bar so, as I have already mentioned, it is best to spend as much time, weakening him, as possible and attack the rest of the monsters only if necessary.
