This page of the Kena: Bridge of Spirits trophy guide explains how to unlock the Piercing Blow achievement. You'll earn the achievement once you kill three enemies with the help of Rot Infused Arrow.

To get this trophy, you'll need to acquire a bow and purchase a skill called Rot Infused Arrow. This skill is located in the Upgrades tab, on the right side.

Once you have a bow and the skill, search for 3 enemies. The easiest way to get this trophy is to approach the weakest / most common opponents. Once you find those, position yourself in such a way that the three enemies will appear to stand very close to each other. The explosion of the Rot Infused Arrow skill should then hit all enemies and kill them.

If you can't inflict enough damage to kill them with one use of the skill, beat them beforehand so they have only a small amount of health left.
