Don't shoot everyone blindly - focus on the drivers or the horses, this will allow you to eliminate a carriage with a single shot. You must eliminate the enemies that manage to jump on your carriage in melee combat. After a few moments Jacob will stop the carriage - quickly leave it and jump to the river.
