One of Chakra Materia can be obtained after reaching Sector 7 Slums, after meeting Avalanche group members. To get it, you need to travel to the abandoned factory.
The above screenshot shows the location of the Chakra Materia. The entrance to the factory grounds is marked with yellow arrow.
Chakra Materia #2 is in the location shown in the screenshot above. Simply pick it up so that it finds its way to your equipment.
