How to unlock: Speak with Haana [Voeld: Resistance Base].
Here you can find the guide for The Lost Scout quest. This is one of the Heleus Assignments (side quests) in Mass Effect: Andromeda game. You can start this quest during your stay on Voeld. In this quest you must find the titular lost scout. You learn that she is imprisoned in the Kett ammo depot that she was sabotaging, and free her.

Unlocking the quest
This quest should be unlocked after spending some time on Voeld. During one of your visits in Resistance Base you can speak with Haana. From this character you learn about the possibility of saving an Angara saboteur imprisoned under rumbles of the blown up Kett ammo depot.
Search the ruins
The ruins are found near the Resistance Base, south-east from it. When you get to the ruined construction leave Nomad and look around the area with scanner turned on. On the first glance, the entrance to the destoryed armory is hard to notice, you can see it in the picture above.
Find the survivors
When you are investigating the ruins you should have your scanner activated. By doing that you can learn that one of the doors is closed. Follow the yellow cables that are visible thanks to the scanner. They lead you to the barricade presented in the picture through which you must go through by using, e.g. a melee weapon. When you go through the barricade you should break into the console to bring the power back.
Free the survivors
Go back to the newly unlocked door and open them. Behind them you can find Mashal, the saboteur that you are looking for, and speak with her. In the same room you can find a heavily wounded Kett. You can allow Mashal to stay in the ruins and continue to torture the Kett or order her to leave this place.
Return to Haana with new information
Regardless of your choice in the ammo depot, you can go back to the Resistance Base and speak with Haana again.
