There are 2 endings to unlock in Dead Space Remake - regular and alternative. On this page of the guide, you will learn how to get the secret ending and when you can unlock it at the earliest.
Main ending
The main ending is identical to the original 2008 version. You get it after dealing with the final boss in chapter 12. Isaac will leave the surface of Aegis VII aboard the shuttle.
Alternative ending
The second, alternate ending is new to the remake of Dead Space. You can't unlock it during the first playthrough.
You must start a new campaign in New Game Plus mode. It will become available in the main menu after finishing the game for the first time. The most important assumptions of New Game Plus are listed on a separate page of the guide.
As you progress through the New Game Plus mode, you can find Marker Fragments. They are a new type of secrets that were not present in the first playthrough. There are 12 fragments to collect and their list can be found in the Secrets and collectibles chapter.
After collecting all 12 fragments, you have to take them to the Office of Captain B. Mathius. It is located in the Executive Quarters section on the Crew Deck. In the office, you will find the Marker surrounded by lit candles. You can leave the collected Marker Fragments here.
After delivering the fragments to Matthius' office, you will advance to the final, 12th, chapter - meaning you will take the shuttle to Aegis VII. You will get the alternate ending instead of the standard one.
