This page of the Diablo 2 Resurrected guide contains information on how to start and complete the Hellforge quest in Act 4.
The Hellforge quest will start in Act 4 when you talk to Decardo Cain. Get to the City of the Damned using the stairs near the portal. Then head over to the River of Flame. The second path will lead to Chaos Sanctuary. This road is guided by angel Hadriel. Here you will also find the boss of this location. Complete this quest before proceeding to the boss fight.
The Smith in Hellforge is not particularly strong, but it has a lot of kin in the front and back of the location, so it's worth pulling them out in small groups.
When you've dealt with the Smith, take the Hellforge Hammer and equip it. Keeping Mephisto's Soulstonearound, go to the anvil and destroy it.
Your reward will drop from Mephisto's Soulstone. This will be gems of the highest class and a random rune.
