How to unlock: Find clues in Adam's house during side quest The Thrill of the Chase.

You receive this quest while completing the side quest The Thrill of the Chase. You need to find clues in Adam's house which suggest that you should explore the quest area. In this particular case, you need to go to the lighthouse in the Eastern part of the map. Follow the hard road towards the lighthouse and come off, after you approach the yellow area marked on the mini-map. You need to locate cave entrance under the lighthouse. It has been shown in the above screenshot and there may be, at least, one toad at the main entrance (e.g. Use the crossbow to kill it quickly).

Go ahead and jump into the water

Enter the cave and reach the ledges. You need to jump off the ledge and jump into a small lake in the cave (make sure that you do not jump into other rocks in mid-air!).

Dive and swim South (while remaining within the yellow area). After you make it to the South, as far as you can get, climb onto the rocks by the water and keep going. The chest is in the spot shown in the above screenshot. There are traps in front of the chest (grenades and tripwires), but it is easy to get around them and jump directly onto the chest. Open the chest and collect the unique revolver (Six-shooter).

The way out of the mine

Although this quest has ended, you still need to leave the cave. Start by jumping towards the ledge above, thanks to which you will avoid the traps. Take a look around the cave and you will find a path up. Keep climbing onto rocks and finally you will reach a spot where you can resurface.
