The Document is in the main part of the prison -at the upper floor, while visiting the control room and opening the cells.

After you have opened the cells at the upper floor, and fought back the mutants, walk over to the third cell to the left - the Document is on the floor.

Document #9

After you leave the forest, you find a big house, to the left of which there is a gate with a lock that you can shoot through. The Document is inside the house, on one of the tables.
