When you get to a Whitechapel, march forward along a main street (If you accost three irrelevant NPCs you obtain another achievement: "A Very Fine Loafer").
Pass three cross streets and you will find a gate of pharmacy you're looking for, nearby the field kitchen. Open it and enter the building.
Talk to the doctor sitting behind a desk. He turns out to be a Watson's familiar, doctor Grant. Ask him about the scalpel.
