After determining that Marsh was hunting people you can decide: send him to prison or let the disease kill him in his own house. After accepting the choice Holmes will leave the closet with a weapon. After the cutscene during which you will learn how Lord Marsh was infected, you must make one more choice: shoot Marsh, shoot Hurst or resign from deciding.
