Decision #1
The first important decision regards Samantha. Either help her burry her father, or stop her from doing this. The difference is whether you'll have the conversation over a grave, or over a corpse of her father.

Decision #2
The second decision regards Alex. You can tell him the truth about the death of his father, or not. Regardless, subsequent events will be identical.
- He's dead - tell him
- He can't come right now - don't tell him
Decision #3
The next one regards the Norma situation. You can show her Randall or hide him until Norma releases your friends. This situation depends on the choice you've made in Episode 2, when you could kill Randall or spare him. If you chose to kill him, he becomes zombie, which kind of makes it impossible to give him back to Norma. Either way, Randall will die, which will cause a fight with Norma.
Decision #4
Another choice regarding Norma. When the Zombies attack, you can leave her to be eaten alive, or kill her and spare her the suffering.
- End Norma's suffering
- Leave Norma to die
Norma dies anyway, so the outcome is identical.
Decision #5
The fifth, at the same time the last, decision of this episode happens in the burning house. You can stay with your daughters (which are only a projection of your mind), or leave them and run.
- Leave your daughters - escape with Samantha
- Stay with them - stay in the burning house
The second choice means that Samantha has to throw you out the window, saving your life. She herself will be consumed by the flames of the fire. The first choice will save both of you.
