How to Get Trembling Hand in Diablo 4
Trembling Hand in Diablo 4 is used to summon Echo of Varshan. Find out how to get this item to farm the boss in our guide.
Echo of Varshan is an important endgame boss in Diablo 4, as its drops are needed to summon Duriel, the only source of Uber Uniques in the game. However, to spawn Varshan, you need to prepare special items as well. One of them is the Trembling Hand. This article explains how and where to obtain it.
How to Get Trembling Hand in Diablo 4
The Trembling Hand is one of the Malignant Body Parts, others being the Gurgling Head, Blackened Femur, and Malignant Heart. You can obtain them from the same sources. The ways to get Trembling Hand in Diablo 4 are:
- Tree of Whispers Cache (from World Tier 3) – complete Whispers of the Dead, which are small objectives marked on the map to collect Grim Favors. Once you have 10 Favors, collect a Cache from the Tree of Whispers. It will contain a random Malignant Body Part (possibly more than one), which may be the Trembling Hand.
- Grotesque Debtor drop – defeating this monster aguarantees a random Malignant Body Part. The Debtors may spawn around the areas of active Whispers of the Dead or in the Bloody Harvest event.
- Transmuting at the Alchemist – you can use 1 other Malignant Body Part, 5 Veiled Crystals and and 1 Abstruse Sigil, 1 Baleful Fragment or 1 Coiling Ward (depending on the Body Part used) to create a Malignant Body Cache with a random drop. You can also use 1 Malignant Heart, 4 Baleful Fragment and 15 Veiled Crystal for a guaranteed Trembling Hand.

Keep in mind that if you want to summon Varshan, you will also need a Gurgling Head, Blackened Femur and Malignant Heart (in World Tier 4), so avoid consuming these materials to transmute Malignant Body Caches unless you have a big stock.
