During the run, you bump into Hossein who is not willing to help Ali and aims his gun at him. In spite of your requests, Hossein does not immediately put his gun away. The situation heats up when Ali decides to produce his gun also.

What is important, from the perspective of the ending, is whether you save Ali or Hossein.

You can try to convince either Ali or Hossein to put the gun away. Regardless of your choice, you will be interrupted by the firing soldiers. You can now save either your cousin or your brother. If you choose Hossein, Ali will bleed out anyway and Hossein does not forget that. It is a good idea to save your brother. He then says thanks you and tells you where the hospital is. Saving either your brother or your cousin determines the ending.

It turns out that Babak got wounded now and it is him that you are escaping with now. Your character completely ignored Ali, but he would die anyway, regardless of your decision.
