After less than two years of break Adam Ryland and developers from Grey Dog Software studio decided to create another game from Total Extreme Wrestling series. Contrary to the earlier version of the cycle, this time the changes were not limited only to details. Developers tried to implement many new elements and significantly modify the game mechanics.

Players again have the opportunity to play the role of manager of a small company involved in the organization and promotion of wrestling fights. Their tasks include hiring and training players, creating duel scripts, negotiating with TV stations, fighting against competition and, of course, taking care of the company's finances.

Among the most important novelties introduced in Total Extreme Wrestling 2010 is the modified user interface. In earlier games in the series, players had to choose action options from drop-down menus. With fans of the series in mind, the creators decided to keep this element, but there is also a much more intuitive, alternative way to control - drag'n'drop.

Other changes include extended user settings, the ability to schedule events in the virtual wrestling calendar, a preview of factors affecting the performance of the action, the use of unclean games during duels, accidental death of players, and an intelligent career end system (wrestlers in good shape can stay longer in the game).

From the visual side of Total Extreme Wrestling 2010 does not differ much from other games in the series. The graphics are still only decorative and the gameplay is based on the text layer (tables, statistics, information, etc.).
